Adoption Agreement

Below is a copy of the adoption agreement used by West End Animal Clinic and Radford Humane Society. This will give you an overview of the guidelines you must follow and information you must provide when adopting a pet through the Radford Humane Society. You will fill out an adoption agreement and pay the adoption fee before obtaining the animal. You can get an adoption agreement at West End Animal Clinic, located at 829 2nd Street in Radford.

Rules:

The RHS retains the right to retake possession of the animal without prior notice at any time it is assessed that the following rules are not being met.

1. Dogs must not be chained/tied outside and denied access to home. Nor must it be left to roam freely through neighborhoods. You must abide by the County Animal Control Ordinance which requires that a dog should be under voice command of its owner at all times.
2. Pet must not be outside in extreme weather conditions.
3. Food and water must be available at all times.
4. Shelter and shade must be available at all times.
5. Pet must be neutered or spayed as soon as it reaches the proper age. Must not be allowed to reproduce or aid in reproduction at any time.
6. Vaccinations must be kept up to date.
7. Medical attention must be provided if necessary.
8. You must be willing to allow a Humane Society Member to inspect the animal at any given time.

If we deem that the above rules are not being complied with we reserve the right to take possession of the animal immediately.
